The hero annoying this villain has a full facemask like Spiderman. It covers his mouth and nose.
Lure him into a narrow area, and fill that area with thugs with lead pipes. LOTS of thugs.
BUT
Every third thug has a spat gun. It shoots what looks like paintballs. They are really a sticky adhesive substance designed to stick to the cloth of his mask.
The goal is to stop his breathing, and to blind him: forcing the hero to remove the mask.
The realization that he is blind and can't breathe will force the hero to hesitate and take the time to remove the mask. [u]For a few seconds the thugs are very effective.[/u]
the splat guns will continue to fire, not as adhesive as previously (Human skin is not a good surface), but effective enough to temporarily blind and inhibit breathing.
The hero will spend more time wiping crap off his face than fighting thugs. Again making the lead pipes very effective.
By the time he gets through this obstacle, the hero ought to be softened up for the super villain minions waiting for him.
OK so how is that for an opening shot?

My heroes seldom cover their mouth and nose, but paint or glue might make Paladin take off his goggles.
While Paladin does have a secret I.D. I'm not sure if it would effect him around a bunch of thugs who don't wear masks.
Paladin can see that none of them is anyone who would recognize him.
One problem with this is it won't effect anyone unless they have a secret I.D. AND want to role play it.
Also what if the character creator does't require us to create a face for under the mask; Then what would we look like when we took our mask off?

I'm not sure about the exact specifics of this scenario because as Paladin said this would only apply to masked heroes who might care about a secret ID. But in more general terms the idea of having debuff type powers in the game that are represented as paintball guns seems very reasonable to me.
IIRC, they actually have things like tear gas paintballs that are used in real life riot control. I could see being hit by a single paintball like this might only apply a very tiny debuff but if you're quickly pelted by several dozen of these kinds of paintballs then the effects could add up and become significant. The game could even come up with multiple types of paintballs like a "glue" type paintball that might provide some kind of Slowing style mez effect.
As far as the mask thing goes I know at least one game (ESO) that has a toggle option for being able to play with your helmet set to being "invisible". What this means is that as far as your in-game appearance goes it looks like you are not wearing anything on your head even though technically you're still wearing your helmet from a full armor stat point of view. The main use for this is to be able to roleplay that you've "taken your helmet off" to be able to talk to each other without having to worry about actually losing any in-game magical effects from wearing your helmet.
I think something like this helmet toggle could be perfectly borrowed for CoT. Yes in CoT we don't have to worry about our costumes helmet/mask giving us any armor or ability stats but from just a pure roleplaying point of view this would allow us to toggle our masks on or off without having to waste an entire costume slot just to accomplish that.

but wouldn't spidey avoid most of those "paintballs" given they are a danger?
in practicality, it would probably work...but in the world of comics it really depends on the writer and what the story is trying to accomplish. Given this, I would make the assumption that most heroes would either flat out avoid them and/or use the attack pipe fellas as cover.

Even if it would work in the world of comics, what villain would do that? It's too much work and too many goons. The more hired help, the greater a chance of failure. Such is why I, the Great Nightmare, have been considering converting to robots in my image.
